IN RE: MINUTE RESOLUTION NO. 05-1028 IN CONNECTION WITH THE GUIDELINES ON RELEASING AND RE-GENERATION OF VOTERS' ID CARDS
Earlier, on 30 August 2005, the Commission pursuant to Minute Resolution No. 05-1028 resolved to defer action on the Guidelines on Releasing and Re-generation of Voters' ID Cards for further discussion by the Commissioner-In-Charge of VVS Project with the members of the Commission. Hence, Commissioner Sadain, CIC of said project and ITD Director Ernesto R. Del Rosario discussed the guidelines aforementioned and submitted the same for the Commission's approval.
After due deliberation, the Commission RESOLVED, as it hereby RESOLVES, to APPROVE the following guidelines on releasing and re-generation of voters ID, provided that the pertinent legal procedures be followed relative to the application for correction, to wit:
- Releasing of ID cards shall be done only at the Office of the Election Officer (EO). No ID cards shall be released at the Information Technology Department (ITD).
- If more than one ID is printed for a voter due to double/multiple records in the database submitted to the ITD, the EO shall determine which ID is valid and issue the valid ID card to the voter. The EO shall likewise delete the invalid record in the local database and inform the ITD of the deletion made through letter.
- Lamination of ID card shall not be allowed in the offices of the COMELEC.
- VRR/ID card of a voter applied for Transfer, Correction of entries, reactivation and validation, approved by the ERB, shall be re-generated by the ITD
- Replacement of ID Card
- If the request for replacement of voter's ID card is due to the wrong encoding of information, replacement shall be free of charge. The following procedures shall be followed in requesting replacement of card:
- Voter shall personally apply before the EO and fill-out application form for Voter's ID card replacement ("Attachment "A")
- Upon approval of the application, voter shall surrender the old ID card to the EO.
- EO shall consolidate requests of ID card replacement and submit the report, using the attached format (Attachment "B"), to the ITD together with the old ID of the voters.
- ITD shall re-generate ID of the voters and transmit to the Office of the EO.
- EO gives the new Voter ID card to the voter
- For lost Voter's ID card or replacement of card due to card face defect, COMELEC shall require the voter to pay P75.00 for re-generation of Voter's ID. The following procedures shall be followed:
- Voter shall personally apply before the Election Officer, fill-out request form for Voter's ID card replacement, and attach the affidavit of lost, in case of lost ID card, in the accomplished request form. In case of replacement due to ID card face defect, old voter's ID card shall be surrendered to the EO
- Upon approval of the request, the voter shall pay corresponding amount to the EO. EO shall issue COMELEC Official Receipt.
- EO shall consolidate request of ID card replacement and submit report to the ITD
- ITD shall re-generate ID of the voters and transmit the ID cards to the office of the EO.
- EO shall give the generated ID to the voter
- All payments received by the EO for the replacement of Voter's ID card shall be forwarded to the Cash Division of the COMELEC
- If the request for replacement of voter's ID card is due to the wrong encoding of information, replacement shall be free of charge. The following procedures shall be followed in requesting replacement of card:
- In case of reports from other parties re fabricated Voter's ID card, the ITD shall verify the validity of the ID card and prepare a certification on the findings of the verification.
Let the Commissioner-In-Charge, Information Technology Department implement this resolution.
